- Surgical Hands-on (Vitreo-Retina):
- Timeline: Specialty hands-on typically begins after the 6th month, following a modular stepwise progression.
- Volume: Described as “more than sufficient.” In a 6-month period, a fellow can expect 600–1,000 retinal lasers and 50–100 intravitreal injections.
- Case Mix: High volume of Vitreous Hemorrhage and ERM (Epiretinal Membrane). Progression to TRD (Tractional Retinal Detachment), RRD, and Macular Hole surgeries occurs once competence is demonstrated.
- The “Amar Agarwal” Philosophy: Because the HOD believes all surgeons should handle complications, hands-on for Secondary IOLs (Glued IOL) and Nucleus Drops is shared across all specialty fellows, not just Retina.
- Surgical Hands-on (Cataract):
- The “Supracapsular” Signature: Fellows are trained in the specific Agarwal technique—prolapsing the nucleus into the anterior chamber before emulsification.
- Note: Many fellows find they need to “re-learn” or maintain their “in-the-bag” technique separately if they plan to practice outside the Agarwal ecosystem later.

- Peripheral Postings: Fellows must be prepared for sudden, short-notice postings to any of Agarwal’s rapidly expanding network of peripheral centers.

PERSONAL OPINION
Agarwal’s Chennai is a “Surgical Finishing School” for Complex Segments. It is the ideal choice for a surgeon who wants to master Medical Retina (Lasers/Injections) and the specific “Agarwal-style” of managing surgical complications like nucleus drops and dislocated lenses. While the ROP exposure is weak and the supracapsular phaco is unconventional, the sheer volume of vitrectomies and the opportunity to train under world-class mentors make it a top-tier destination for those looking to join a high-end corporate or surgical practice.
