- Selection & Intake: Selection is via a personal interview. The department typically inducts 2 VR fellows and 1 Medical Retina (MR) fellow every 3 months, ensuring a steady rotation of trainees.
- Surgical Hands-on (The “Aravind” Ladder):
- Phase 1 (Medical Retina): Months 4–7 are dedicated to high-volume diagnostics and minor procedures. You will perform “copious numbers” of B-scans, retinal lasers, and intravitreal injections.
- Phase 2 (Surgical Steps): Training in the OR typically begins around the 7th or 8th month.
- Independence: Highly skill-dependent. While some fellows with “good hands” may perform simple cases independently within the first year, others may only perform surgical steps.
- Macular Surgery: Exposure is more conservative here than at other branches. Independent ILM (Internal Limiting Membrane) peeling is rarely given, often limited to 1–2 “token” steps in the final month.
- Cataract Training: None. This is a pure specialty track.
- Clinical Exposure:
- Volume: Extremely Hectic. The department sees an average of 400–500 patients daily.
- ROP: Training occurs in the last 3 months, focusing strictly on screening. No hands-on for ROP lasers or pediatric injections is provided.
- Work Culture & Schedule:
- Hours: The most demanding in the hospital (7:00 AM – 8:00/9:00 PM). VR fellows are consistently the last to leave.
- Duties: Night calls once a month; Sunday duties once every 6 months. However, you will likely rotate for “Trial Bandage” duty on other Sundays (screening camp patients for surgery).
- Academics: Strong academic culture with regular classes and grand rounds.
PERSONAL OPINION
Aravind Chennai is a “Clinical Endurance Test.” While the surgical numbers for complex cases (like macular holes) are reportedly lower than at Aravind Madurai or Coimbatore, the Medical Retina volume and diagnostic training are top-tier. It is the ideal program for someone who wants to be “baptized by fire” in a high-volume urban setting. Many graduates choose to stay on as Medical Officers (MOs) to bridge the gap between “step-surgeon” and “independent surgeon,” which is a common pathway in this branch.
