- Selection Process: Includes a 2-day interview-cum-observation period where candidates are evaluated on clinical acumen and basic surgical orientation.
- Orientation: A mandatory 1-month orientation course precedes the core fellowship.

- Structure: 3 Months General Ophthalmology Posting + 21 Months Core Glaucoma Department.

1. Surgical Hands-on (The “Cutting” Advantage)
- Cataract Volume: You are guaranteed an unlimited number of SICS procedures. This builds an exceptionally strong foundational surgical “hand.”
- Glaucoma Triple Procedures: You can expect to perform roughly 75–80 SICS + Trabeculectomy (Combined) surgeries.
- Phacoemulsification: Phaco training typically starts in the last 3 months. Fellows generally complete 50 Phacos (including Phaco + Trab combinations).
- Advanced Procedures: * Lasers: High volume of Nd:YAG Peripheral Iridotomy (PI).
- Advanced Tech: Exposure to SLT (Selective Laser Trabeculoplasty) and MIGS is provided in the latter half of the fellowship, contingent on individual surgical proficiency.
- GDD: Initial hands-on exposure to Glaucoma Drainage Devices (Valves) is part of the senior rotation.
2. Clinical Exposure & OPD
- Patient Load: Extremely Heavy. The OPD volume ensures that you encounter every clinical variation of glaucoma—from primary open-angle to complex secondary and pediatric glaucomas.
- Diagnostics: Mastery of Gonioscopy, Applanation Tonometry, and Visual Field (HFA) interpretation is a core outcome of the program.
3. Work Culture & Duties
- Intensity: Can be hectic. Standard duties include:
- Outreach Cataract Camps.
- Post-operative cataract surgery rounds.
- Night call duties.
- Academics: Decent and structured. Includes bi-weekly journal clubs and regularly scheduled specialty classes.
- Location Note: Tirunelveli is a remote location compared to Chennai or Coimbatore. The scope for “personal enjoyment” or nightlife is limited; the focus here is strictly professional immersion.
PERSONAL OPINION
Aravind Tirunelveli is the “Surgical Powerhouse” for Glaucoma. If your goal is to graduate as a surgeon who can perform a Trabeculectomy or a combined procedure with their eyes closed, this is the best center in the Aravind network. While the Phaco numbers are modest, the SICS and Trab volume is unrivaled. It is a “monastic” fellowship—best for those willing to trade two years of city life for elite surgical expertise.
