STIPEND & SELECTION
- Monthly Remuneration: ₹40,000 (Year 1) | ₹50,000 (Year 2).
- Selection Process: National entrance exam followed by a personal interview and a clinical/surgical observation period (Trade Test).
- Intake: Multiple fellows are inducted in staggered batches (January and July cycles).
DURATION
24 Months (2 Years)
- Structure: 1–2 months General Ophthalmology + 1 month Uvea + 21 months core Vitreo-Retinal posting.
HONEST REVIEW
1. Surgical Hands-On (The Modular Climb)
- Timeline: Specialty surgical training typically begins around the 4th month. The early phase is strictly observational and focused on assisting.
- Surgical Volume: Fellows typically perform 200–250 VR procedures independently by the end of 24 months.
- The “Step” System: You are trained in modules—starting with port making, fluid-gas exchange, and endolaser, progressing to membrane peeling and retinal detachment repair.
- The MO Advantage: Because the volume of complex cases (PDR, Macular holes) is so high, many fellows choose to stay as Medical Officers (MOs) for an additional year to gain the high-speed surgical independence that the fellowship’s “supervised” nature may not fully provide.
- Cataract Training: None. Phaco is not part of the VR curriculum. If you need cataract volume, you must be proactive during your initial General Ophthalmology rotation or serve in a peripheral center.
2. Clinical Exposure (Textbook Pathology)
- Variety: Unrivaled. You will see every retinal condition imaginable, from common diabetic complications to rare genetic dystrophies and ocular oncology cases managed by world-renowned surgeons.
- Diagnostics: Mastery of Indirect Ophthalmoscopy is mandatory. You get extensive hands-on with Multimodal Imaging (FFA, ICG, OCT-A, and high-frequency B-scans).
3. Work Culture & Duties
- Environment: Extremely Hectic. The patient load is relentless, but it is described as “educationally rewarding.”
- Mentorship: There is no “one-on-one” mentor. You rotate through various units, learning different surgical styles from a pool of senior consultants. Self-initiative is the primary driver of your progress.
- Calls & Camps:
- Night Duty: ~Once every 2 months.
- Sunday Duty: ~Once every 3 months.
- Camps: Not routine for VR fellows (mostly handled by IOL fellows).
PERSONAL OPINION
Aravind Madurai is the “Retina University.” If you want to be a master of medical retina and a protocol-driven surgeon, this is the place. While you might not get the “independent” surgical freedom found in smaller private setups early on, the diagnostic discipline and volume of Retinal Lasers you perform here will make you a formidable clinician. It is best suited for those who are willing to commit to a 3-year journey (2 years fellowship + 1 year MO) to truly master the craft.
