DURATION
24 Months (2 Years)
- Intake: One fellow is typically inducted every 3 months, ensuring a staggered peer-mentorship model.
HONEST REVIEW
- The “Frustration” Phase (Months 1–6):
- Training begins with a 3-month clinical period. OT starts thereafter, beginning with 10 supervised SICS in a dedicated training OT.
- Initially, you get 2 OT turns per week, but these can be irregular if you are pulled to cover busy OPDs or outreach camps. This phase is described as “frustrating” but a standard part of the Aravind “grind.”
- Surgical Hands-on (The “Volume” Promise):
- SICS: You are promised 1,000 cases, but most fellows comfortably perform 2,000+ SICS over two years.
- Phacoemulsification: Training officially begins in the 2nd year. While turns may be irregular initially, the volume ramps up significantly in the last 6 months. You can expect to cross 100+ Phaco cases (primarily Grade 2 nuclei).
- Limitations: Advanced Phaco cases and independent management of complications (like anterior vitrectomy) are generally not provided during the fellowship. These are reserved for those who stay on as Medical Officers (MOs).
- Clinical & Specialty Exposure:
- OPD: Extremely hectic. You are expected to be a “team player,” often assisting in other departments during surges.
- Rotations: Includes 1-month specialty postings in the 2nd year. Depending on your proactiveness, you can gain hands-on for intravitreal injections and YAG PI.
- Work Culture & Duties:
- Timings: 7:30 AM – 6:00 PM (Strict and disciplined).
- Calls: 1 Night duty per month; 1–2 Outreach camps per month; Sunday duties are rare (once every 2+ months).
- The “MO” Advantage: Coimbatore is noted for having a very supportive OT staff and senior mentors who push MOs to master advanced surgeries more aggressively than at other Aravind branches.
PERSONAL OPINION
Aravind Coimbatore is a “Surgical Marathon.” It is the ideal program for a surgeon who wants to achieve absolute mastery over SICS. By the time you finish 2,000 cases, your tissue handling will be instinctive. While the Phaco volume is lower than at corporate centers, the predictability and safety of the Aravind system are unmatched. If you plan to join a high-volume charitable hospital or start a mission-based practice, this is the gold standard.
